Coach Disbursement Request

Tournament proceeds can be used for the following without exception:
    1. Tournament entry fees
    2. Equipment such as team bats, helmets, catchers equipment, training tools/aids, and
    3. Purchase of team uniforms and hats
    4. MYAS league fee summer or fall leagues
    5. Umpire fees

You can access the funds allocated to your team by using one of the following methods:
    1. Complete the Coaches request form ** attached at the bottom of this page **
    2. Email the form to

    4. Fax the form to 320‐214‐0000
    5. Mail the form to: Willmar Baseball Association, PO Box 754, Willmar MN 56201
    6. Coaches may be reimbursed for expenses paid for out of pocket. To be reimbursed you
        must send a copy of the cancelled check (front and back) AND a receipt or tournament
        entry form. No reimbursement will be paid out if you fail to provide the required

Checks will be made available within 7 days from the receipt of the request. You will be
notified by email when your check is ready.

If your team does not use all of the current year’s available funds they can be carried over to
the next season.
